You mentioned severe anxiety and depression. Have you been tested for Lyme disease and all of its co-infections? They can cause all kinds of psychiatric illness, including severe anxiety and depression, and these infections are VERY OFTEN misdiagnosed (often for years and years) as CFS and/or FM. Just a thought - something to investigate if you haven't already.
